CLSC (formerly Intermed Clinics) is a family-run business founded by Dr. Jean-Claude Bourque and his daughter Chantal Bourque-Bray, M.H.S.A. Sophie Bray joined our team later, bringing her expertise as CFO and PCMH Manager. With a commitment to treating our office like family, we’ve been dedicated to delivering compassionate healthcare since 1989.
Our South Florida facilities are located in Broward and Palm Beach County – with offices in Hallandale, Hollywood, Boca Raton, and Coconut Creek. In 2024, CLSC expanded into Central Florida with an office in Osceola County in the St. Cloud area.
Our team includes 23 providers, featuring five advanced practice nurse practitioners, eight doctors, and ten physician assistants, fluent in a multitude of languages to better serve our community.
